Schedule - latest listing of dues and fees charged to member and non-member

Application for Candidacy - please contact the Commission Office for access to the
electronic version of the Candidacy Application form
Act 48 Forms - service
on a visiting team and attendance at workshops makes Pennsylvania
certified teachers eligible for Act 48 credit hours; use this
form to request submission of your hours
Annual Profile - member
schools are required to update their statistical information
each year; you can login here to submit the data electronically
Application for
Evaluation - in September the latest version of the Application
for Evaluation is mailed to schools entering their self study year (i.e. the
school year before the one in which an accreditation visit is
scheduled); applications are only processed upon payment of
the Evaluation Preparation Fee
Fact Sheet -
provides the current statistical data on CSS and its member institutions.
First Request for
Extension - member schools requesting a one year extension of
their accreditation (a postponement of a visit for one year) must complete this
form
Second Request for
Extension - member schools with extreme circumstances may request a
second extension. Requests for a second extension must be accompanied by a
completed Assessment of Compliance
with the Standards. This 31-page Word document may be downloaded.
Notice of Intent to Evaluate - a Word document version of the form member
schools are sent with their Date Confirmation letter, for ease of editing.
AFG Commitment
Form - member schools engaged in the
Accreditation for Growth protocol should submit this form with
at least two signatures early in the self study process
VTV Commitment
Form - member schools engaged in the Validating the
Vision protocol should submit this form with at least two signatures
early in the self study process
Nomination Form - member
schools are asked to nominate staff members who are willing
and able to serve as visiting team members for the fall and
spring teams of the next year; initial requests are mailed
to school heads in late spring, but late submissions or updates
to this interactive form are always welcome; a member login is required.
Team Chair Evaluation of Team Survey. Chairs of visiting teams are requested to complete this survey to evaluate their team experience and the contributions of the members of their team following the visit. This is a link to an online survey.
Team Members Evaluation of Team Experience Survey. Members of visiting teams are requested to complete this survey to evaluate their team experience following the visit. This is a link to an online survey.
School’s Evaluation of Team and Accreditation Experience. Heads of schools are requested to complete this survey of evaluate the support they received from CSS and the quality of the team visit and team report following the visit. This is a link to an online survey.
